Health + Housing (medical respite/supportive housing) Coalition

Healthy housing advocates are invited to attend our quarterly coalition meeting.
Our vision: Address the challenges medically complex homeless individuals in the Omaha Council Bluffs Metro face through the design of a continuum of services that identifies frequent users of healthcare systems and emergency shelters and connects them into the appropriate level of care.
—Engage in shared learning around health and housing, national models and/or best practices for addressing housing stability within the medically complex, chronically homeless population—Recruit additional partners when applicable—This group received funding to pilot medical respite in the Omaha Council Bluffs Metro Area, with a planned launch date of July 2022.
